CLASS "A" FAST PITCH NORTHERN NATIONAL
CHAMPIONSHIP

REQUIRED PRE-TOURNAMENT PAPERWORK

All managers or their business managers need to provide the following information before their team is officially in the tournament.

The original tournament entry form from your state/metro commissioner. This entry form MUST be signed by either your state/metro commissioner in order to be an official entry form. Original copy is requested to be in ASA Metro St. Louis office not later than 5:00PM on 22 July 2008.

A check or money order made out to Metro St. Louis ASA in the amount of $350.00 to cover team tournament entry fee should be mailed as soon as possible. Do not send via regular mail after 17 July 2008 or express mail after 20 July 2008. Failure to provide check by 5:00PM on 22 July 2008 could result in your team not being entered in the tournament.

    The original copy of the team's ASA Championship Roster entitled "ASA Official Wavier & Release of Liability & Indemnification Form" must be completely filled out with all the required information and signed in accordance with ASA requirements including team manager's signature, the ASA Local Commissioner or designee and the ASA Deputy/District Commissioner. Electronic reproduction or a carbon copy is NOT acceptable unless the electronic reproduction or carbon copy is legible and has original signatures for the team manager, the ASA Local Commissioner or designee and the ASA Deputy/District Commissioner. It should be noted that in some cases the signature for the ASA Local Commissioner or designee and the ASA Deputy/District Commissioner maybe the same. This depends on the organizational structure and operational procedures of each ASA Local Association. The roster must be RED LINED following the last player listed on the roster. Original copy of the completed ASA Championship Roster is requested to be in the Metro St. Louis ASA office not later than 22 July 2008.

   Each team is requested to submit the following team information as some as possible to the Per Tournament Planning Coordinator or FAX 314-429-1705: name of team manager; name of team; home town of team; email address of team manager; home and business phone number of team manager; cell phone of team manager; name of team coaches and name, playing position, and number for each player on team. This information will be used to contact the team manager if necessary and provide information for the tournament program.

    If the above information is not provided a team risks the possibility of not being entered in the tournament and there will not be any communication information available to the tournament committee to contact with your team.

    The submission of player date of birth verification information is optional as part of the pre-tournament paper work. However, if this information is not provided as part of the pre-tournament paper work the team check in process will take much longer. If this information is provided early it must be in the Metro St. Louis ASA office not later than 24 July 2008 to avoid a time consuming team check-in.
